The Department of Medicine, Division of Geriatrics Medicine at Bayview Medical Center is seeking a Nurse Practitioner. The Nurse Practitioner provides primary medical care for Hopkins ElderPlus participants. All participants are certified as requiring a nursing home level of care and are typically elderly, frail and low-income. Hopkins ElderPlus receives capitation payments from Medicare and/or Medicaid, and assumes full financial risk for care provided to its participants.
The Nurse Practitioner will collaborate with and support the Hopkins ElderPlus Interdisciplinary Team, which operates according to the guidelines established for the Program of All-Inclusive Care for the Elderly (PACE). The goal of PACE and Hopkins ElderPlus, is to assist participants to remain in their own homes and communities as long as possible.
Principal Duties and Responsibilities: Provides acute and chronic medical care to frail, older adults in the: Hopkins ElderPlus Keyser Clinic; participant's home; assisted living facility; hospital and nursing home.Works collaboratively with the medical director and Interdisciplinary team (IDT).
Essential Job Functions: Performs routine scheduled, urgent, and emergent medical evaluations. Participates in interdisciplinary team meetings and represents the medical perspective. Orders laboratory and diagnostic studies. Prescribes medications and treatments. Coordinates and/or provides care across or in multiple settings including nursing home and hospitals. Coordinates medical consultations. Arranges for medical follow-up during transitions of care. Continuously communicates with IDT members, family and other caregivers in all settings.
Participates in the full range of quality improvement activities including taking a leadership role as directed. Serves as a clinical preceptor for students enrolled in approved universities. Provides direct instructions to students, residents, fellows regarding PACE and the interdisciplinary team. Assists in recruitment and completion of study forms for public and private sector research protocols as requested.
Completes written Â·documentation required by all regulatory agencies. Coordinates and monitors performance of assigned students and volunteers. Maintains records and collects statistical data to assist with planning, evaluation and fiscal management of program.
May participate on Hopkins ElderPlus committees such as Medical Records, Utilization Review, Quality Improvement, etc. May participate in JHBMC, JHU and JHHS committees as directed or permitted.
Master's degree in nursing; Registered Nurse license and Certified as a Nurse Practitioner. Must be licensed as NP in State of Maryland or other state where practicing. Minimum of three years professional experience as a CRNP working with a frail, elderly population is strongly preferred. Current CPR certification required. Must maintain current licensure and certification during duration of employment. Completion of the JHH, Bayview Medical Center credentialing process is required prior to start date.
Additional Knowledge, Skills and Abilities: Work requires the analytical skills necessary to resolve problems requiring a professional level of knowledge in a specific discipline/field and/or improve, enhance or upgrade complex clinical, financial, data processing, marketing or human resources systems and programs.
Work requires the communication skills necessary to effectively collaborate, communicate with and support the IDT and all levels of staff in the delivery of quality medical care. Work requires effectively dealing with conflicting views or issues and implementing care plans and tasks developed by consensus of the IDT.
Must complete the required continuous education requirements to maintain license. Must maintain prescriptive authority and collaborative agreement as required under state law.
Must complete all training required to meet state and federal regulations as applied to PACE and components of PCE, such as adult day living centers, home care and care management.
Must maintain a current driver's license. Must have reliable access to an automobile for routine and urgent travel.
Physical Requirements: Work requires occasionally reaching and grasping objects. Work requires occasionally using a keyboard to interact with computer system. Work requires creating, proofreading and /or reviewing documents. Work requires direct, hands-on care of frail elderly participants and the diagnosis and treatment of illness and injury, with potential for exposure to communicable disease.
Working Conditions: Work is generally performed in a clinical environment in which there is only minimal exposure to unpleasant and/or hazardous working conditions. Work may be performed in participant's homes, where conditions may be unpleasant and insecure.
